Why do all subscriptions use estimated usage like ~500 chat messages?

That's because CodeNext charges based on token usage, not numbe of messages/requests. Thus all the numbers are estimates based on average usage.

Does CodeNext require any special permissions from MacOS?

Yes, CodeNext requires access to accessibility and folder access permission. Please note that the accessibility permission is solely used to extract and insert content from/to Xcode.
